Üremik Pruritus ve Hemşirelik Yönetimi

Abstract

Üremik pruritus, başka bir nedene bağlı olmadan son dönem böbrek yetmezliğinde görülen kaşıntıdır. Sıklıkla diyaliz öncesi sırt, karın, baş ve kollarda görülen, yaşamı tehdit etmeyen, ciltte kaşınma, çizme isteği yaratan hoşa gitmeyen bir duygudur. Ayrıca bireyin yaşam kalitesini düşüren bir semptomdur. Bu özellikleriyle hastayı biyopsikososyal yönden olumsuz etkileyen ve kontrol altına alınması gereken bir durumdur. Üremik pruritusa yönelik yapılan tedaviler semptomatiktir ve hemşirelik yönetiminde hastanın izlemi, hasta ve hasta yakınlarının eğitimi, cilt ve tırnakların bakımı, dikkati başka yöne çekme teknikleri, aromaterapi, akupunktur-akupressör, masaj ve müzik gibi bütünleşik uygulamalara yer verilmektedir. Bu derleme üremik pruritusu olan hastanın hemşirelik yönetimi ve bu yönetim sürecinde yer alabilecek bütünleşik yaklaşımlar hakkında hemşirelere rehberlik sağlamak amacıyla hazırlanmıştır.

Uremic pruritus is an itch in end-stage renal failure regardless of any other cause. It is an unpleasant feeling, that often appears on the back, abdomen head and arms before dialysis, not life threatening, creating itching and scratching on the skin. It is also a symptom that drops an individual’s quality of life. With these features, it is a condition that affects the patient negatively in terms of biopsychosocial and should be taken under control. Treatments for uremic pruritus are symptomatic. Patient monitoring, education of patients and patient relatives, skin and nail care, techniques to draw attention to another direction, aromatherapy, acupuncture-acupressor, massage and music are integrated applications of nursing management for uremic pruritus. This review was indited to provide guidance for nurses on the nursing management of the patient with uremic pruritus about integrated approaches that can take place in this management process.
